An immediate posterior replacement bridge based on implants
This case features a situation where a failing tooth-supported bridge was immediately replaced by a bridge supported by Conelog® implants. Conelog® implants were selected because their conical connection allows for a slight divergence in paths of insertion for a screw-retained bridge. In addition, they have a very strong and durable Titanium temporary post.

This shows how a case with failing maxillary dentition is handled from extractions, to bone reduction, implant placement, bone grafting and the placement of Camlog® Vario SR abutments. These are then linked to sleeves housed in a denture to give an immediate screw-retained All-On-Six fixed hybrid reconstruction.

Dentistry is moving into the digital world and using these tools it is possible to plan and execute surgical guide placement of implants and then to place provisional restorations. See how a very skilled operator managed to accomplish all this in one of his earliest implant procedures.

This is the companion video to the program "Immediate replacement of a primary canine". It shows how the abutment and provisional crown were planned "virtually" before being exported for milling. This was planned out following the generation of the implant channel by drilling into the model using the Surgical Guide. This generated the implant analog in the position of the proposed implant. All this allowed the provisional restorative components to be ready for seating when the real implant was placed.

It's common enough, a removable partial denture destroys the adjacent natural teeth. In this case it is the maxillary incisors that are suffering. The patient wants something more stable, healthy and aesthetic. This is supplied by a digital driven approach where a Surgical Guide, Zirconia Emergence Abutments and a Provisional bridge are prepared before the procedure. See also the second part of this presentation (From Ridge to Bridge : Part 2 - Clinical procedure).

This is a follow-up from Ridge to Bridge Part 1. This shows how the clinical procedure goes. She has the two lateral incisors removed, implants are placed using a Surgical guide. Then it's possible to insert some provisional zirconia emergence abutments and a provisional bridge. The partial denture is thrown away and the patient is delighted.

A maxillary molar is failing and the quadrant will then have no molars and only one premolar remaining. The solution is to remove the failing molar and to place an implant immediately with a sinus lift and socket augmentation at the same time. In the edentulous premolar space further forwards another implant was placed. Later, these implants can both be restored with individual restorations.

Everything seemed to be failing. Teeth, implants, bite collapse; it looked as though he was moving towards full dentures. By a careful analysis of the issues, with removal of the factors causing breakdown we were able to bring him under control. We then moved him to a durable set of provisional restorations which allowed a transition from tooth supports to implant supports. It became possible to make him stable, functional and comfortable.
